PREDATOR ATHLETICS WAIVER OF LIABILIAY, INDEMNIFICATION, ASSUMPTION OF RISK, AUTHORISATION TO CONTACT EMERGANCY MEDICAL ASSISTANCE, PUBLICITY & WHATSAPP RELEASE FORM

 (collectively the “Participant Agreement”) 

I herby acknowledge the risks involved when taking part in cheerleading and myself/my child is able to use NHS medical care in the unlikely event of an injury or accident when taking part. I authorise Predator Athletics staff to contact emergency services on mine/my child’s behalf should the need arise (including but not limited to serious illness or injury). I also hereby release and absolve Predator Athletics, its subsidiaries, staff, employees, and directors from all liability and responsibility for injuries, sickness (including but not limited to Covid-19), accidents, loss of money and property that may be sustained whilst participating at Predator Athletics classes, training, events, try-outs, workshops, camps and/or competitions. 

In consideration of myself/ my athlete training at Predator Athletics, I am allowing myself / my child, to participate in Predator Athletics training and or events as above and intend to be legally bound and agree to waive and release all rights to claim for damages which I or my child may sustain or suffer whilst participating at classes, training or an event, including travelling to and from training or an event.

I also confirm that I / my child, have not been advised by a medical professional to avoid physical exercise or cheerleading and do not know of any problems that may adversely affect my / their health when taking part at training/events. Should Predator Athletics staff feel in their professional opinion that I am not physically able to participate, I / my child will cease participation. 

I also give permission for myself / my child to be photographed, video or audio-taped during any of the Predator Athletics events/training, and give permission for such photographs, video and audio tapes to be used in print or broadcast through any media which is deemed appropriate for the promotion of Predator Athletics activities, promotions and publicity.

I also give permission for myself/ my athlete to be added to my designated teams WhatsApp group if/when they move up into competitive teams and confirm that I have access to this app for program communication purposes.

By accepting our terms and conditions I certify that: (1) I acknowledge and have fully and completely read, accept agree and understand this participant agreement; (2) I am 18 years of age or older (3) I am the participant/ the parent or legal guardian of the participant and have the legal right to sign on the participants behalf; (4) I consent and agree to all of the foregoing on behalf of the participant.
